Consumer Health Data Privacy Policy

This Consumer Health Data Privacy Policy (“CHD Policy”) applies to Suncoast Trim Clinic LLC (“we,” “us,” or “our”) and supplements our general Privacy Policy.
This policy is provided to comply with the Washington My Health My Data Act (MHMDA), Nevada Consumer Health Data Law, and the Connecticut Data Privacy Act (CTDPA). It explains how we collect, use, share, and protect Consumer Health Data (“CHD”) and describes your rights related to such data.
If there is a conflict between this CHD Policy and our general Privacy Policy, this CHD Policy governs with respect to Consumer Health Data.
1. What Is Consumer Health Data (CHD)
Consumer Health Data includes personal information that is linked or reasonably linkable to an individual and identifies past, present, or future physical or mental health status.
CHD we may collect includes, but is not limited to:
- Health conditions, symptoms, diagnoses, and treatment interests
- Weight, body composition, obesity or weight-related information
- Medication-related information (including GLP-1/GIP medications such as semaglutide or tirzepatide)
- Intake forms, consultation notes, care plans, and communications
- Appointment scheduling and care coordination details
- Communications with clinicians or care team members
- Information derived from interactions that may indicate a health condition
2. Where We Collect Consumer Health Data
We may collect Consumer Health Data from:
- You directly (via website forms, intake questionnaires, telehealth visits, messages, or calls)
- Licensed healthcare providers involved in your care
- Partner platforms used to deliver services (e.g., telehealth platforms, scheduling systems)
- Pharmacies, laboratories, or fulfillment partners when applicable
- Payment or financing partners where relevant to care
- Cookies or analytics tools only when related to health-related site interactions (such as intake or booking pages)
3. Why We Collect and Use Consumer Health Data
We collect and use Consumer Health Data to:
- Provide medical, telehealth, and wellness services
- Screen patients, conduct consultations, and manage care
- Coordinate prescriptions, supplements, and treatment plans
- Communicate appointment reminders and care instructions
- Operate, maintain, and improve our services and Website

We do not sell Consumer Health Data and do not use Consumer Health Data for targeted advertising purposes.

We do not share Consumer Health Data for targeted advertising.
5. Consent & Withdrawal (MHMDA / Nevada / CTDPA)
Where required by law, we obtain your consent before collecting or sharing Consumer Health Data for non-care purposes (such as analytics unrelated to treatment or care).
You may withdraw consent at any time by:
- Using our Privacy Choices tools, or
- Contacting us directly (see Section 10)
Withdrawal of consent does not affect lawful processing already performed.
6. Your Rights Related to Consumer Health Data
Subject to applicable law and certain exceptions, you may have the right to:
- Confirm whether we collect or share your Consumer Health Data
- Access your Consumer Health Data
- Correct inaccuracies
- Request deletion of Consumer Health Data (subject to medical and legal retention requirements)
- Withdraw consent for collection or sharing of Consumer Health Data

Appeals:
If we deny your request, you may appeal our decision. Depending on your state of residence, you may also contact your state Attorney General, including:

We use cookies and similar technologies to operate and improve our Website (performance, security, usability).

We use reasonable administrative, technical, and physical safeguards to protect Consumer Health Data and limit access to authorized personnel.
We retain Consumer Health Data only as long as necessary for care, legal, regulatory, and operational purposes, after which it is securely deleted or de-identified.

11. Partner Platforms & Independent Providers
If you use services delivered through a partner telehealth platform or independent provider network, your Consumer Health Data may also be subject to that platform’s privacy policies and terms, in addition to this Policy.
12. Minors
Our services are intended for adults 18 years of age or older. We do not knowingly collect Consumer Health Data from children under 13. If you believe such data has been collected in error, please contact us for deletion.
13. Geofencing (MHMDA)
We do not use geofencing technology to identify, track, or target individuals near healthcare facilities for purposes restricted under the My Health My Data Act.
14. Changes to This Policy
We may update this Consumer Health Data Privacy Policy as laws or practices change. The “Effective Date” above reflects the most current version. Continued use of our services after updates constitutes acceptance.
