REFERRAL PROGRAM TERMS

Effective Date: September 22, 2025

Suncoast Trim Clinic LLC (“Suncoast Trim Clinic” or “STC”) offers a voluntary referral program to thank active members who share information about our telehealth coordination services with others.

Participation in the referral program constitutes acceptance of these terms.

1. Eligibility

The referral program is available only to:

  • Current Concierge Support Membership participants in good standing
  • Individuals age 18 or older

The referred individual must:

  • Be a new patient who has not previously received services through STC
  • Independently choose to enroll without coercion or obligation
  • Complete a telehealth evaluation
  • Receive a prescription from a licensed healthcare provider
  • Begin an active paid membership cycle

Employees, contractors, affiliated medical providers, and their immediate family members are not eligible to participate.

Self-referrals are strictly prohibited.

This program is void where prohibited by law.

STC reserves the right to exclude individuals receiving government-funded healthcare benefits where required by applicable federal or state regulations.

2. Referral Credit

If the referred individual:

  • Is prescribed treatment by a licensed provider, and
  • Successfully completes their first paid 28-day membership cycle without refund, reversal, dispute, or chargeback

Then:

  • The referring member will receive a $25 promotional account credit
  • The referred member will receive a $25 promotional account credit

Credits are issued only after completion of the qualifying billing cycle and confirmation of account good standing.

3. Nature of Referral Credits

Referral credits:

  • Are promotional in nature
  • Have no cash value
  • Are non-transferable
  • May not be redeemed for cash, refunds, gift cards, or cash equivalents
  • May only be applied toward eligible STC membership fees
  • May not be applied toward prescription medication costs
  • May not be combined with other promotional offers unless explicitly permitted

Credits are forfeited if either party is not an active member in good standing at the time of issuance.

4. No Medical or Clinical Incentive

Referral credits are offered solely as a promotional courtesy related to administrative membership services.

Referral credits:

  • Are not compensation for medical services
  • Do not influence clinical decision-making
  • Do not guarantee eligibility for treatment
  • Do not guarantee issuance of a prescription
  • Do not alter provider medical judgment

All medical decisions are made independently by licensed healthcare providers based solely on clinical appropriateness.

5. Compliance With Healthcare Laws

This referral program is structured to comply with applicable federal and state laws governing healthcare marketing and patient inducements.

Nothing in this program is intended to:

  • Induce the use of medically unnecessary services
  • Influence provider prescribing decisions
  • Violate federal Anti-Kickback Statute, state inducement laws, or similar regulations

STC reserves the right to modify eligibility criteria or deny participation as necessary to ensure legal compliance.

6. Anti-Abuse & Program Integrity

STC reserves the right to:

  • Verify the legitimacy of all referrals
  • Deny or revoke credits in cases of suspected fraud, abuse, manipulation, or artificial account creation
  • Cancel credits issued in error
  • Suspend or terminate accounts engaging in referral abuse
  • Modify, suspend, or terminate the referral program at any time without prior notice

Referral abuse includes, but is not limited to:

  • Creating duplicate accounts
  • Coordinated billing manipulation
  • Self-referrals
  • Misrepresentation of identity

7. General Disclosures

Participation in this referral program:

  • Is voluntary
  • Is not required to receive services
  • Does not establish a provider-patient relationship
  • Does not guarantee continued availability of services

Suncoast Trim Clinic is a telehealth facilitation and administrative support platform and does not practice medicine.

Medical services are delivered by independently licensed healthcare providers.
